Simply put, scoliosis is a bend or cuve in the spine whereas a healthy spine would be completely vertical.  The exact origin of the curve is difficult to trace.  In the beginning stages it may just be a small bend caused by a slight misalignment of a single vertebrae. Over the course of time the body will begin to over-compensate, building musculature that will continue to pull the spine into lateral curves to try and keep the head level.  

 Typically, the misalignment occurs because of postural issues, traumatic injuries, or degenerative diseases.  While many cases cause scoliosis to start when your body is going through growth spurts, particularly in youths 10-12, the condition can also begin in older adults as well.  In older people, scioliosis arises primarily from degenerative disorders such as osteoporosis.

Parents typically noticed scoliosis as the shoulders and pelvis no longer appear level.  A physical examination can assess the issue and visual examinations including x-rays can asses the spine curvature.  An "Adams Position Test", which requires the patient to bend forward as far as they possibly can, can indicate whether an issue is present.  If a noticeable curvature is discovered then most likely an x-ray will determine the stage of scoliosis.
